You're in great hands

We make sure all of our patients feel welcome, safe, and taken care of. During your first visit, we will review your medical and surgical history. For initial diagnosis, a thorough vascular exam will be performed and vascular ultrasound studies may be ordered to be done at a later date. From there, we will help you choose the best option for treatment of your vascular disease. We typically proceed with the least invasive option as our first line of therapy.

For a shorter time in the waiting room, please download the New Patient Form, Consent Form, Pharmacy Form, Records Release Form, and Leave Message Form. Please fill it out all the forms before your visit and bring other x-rays, reports, etc. as explained in the cover letter. Note that each PDF file will download onto your computer to wherever you save downloaded files.

 

To ensure that your visit with us is as smooth and timely as possible, please bring the following items with you. This will help to avoid any cancellation or rescheduling of your appointment.

1.    All patient forms listed above - these forms must be completed in full prior to check-in, if not, your appointment may be significantly delayed or cancelled.

2.     Your insurance card(s).

3.     Please bring pertinent test or x-ray results that have been done at another facility, including but not limited to CDs of the MRI/MRA, CT scan or angiogram films.

  • Please check-out and pick-up outside CDs and bring them with you to your appointment

4.     Authorization for your visit (as applicable).

5.     Co-payment (as applicable).

If you are having a vascular lab study or having vein injections, you may want to wear or change into a short sleeve shirt/top, shorts or other loose-fitting clothing for that portion of your visit.

Should there be a need to cancel or to reschedule your current appointment, we ask that you kindly give us at least 24-hour notice. Again, thank you for selecting our office for your vascular and endovascular surgery needs.
